CSTADS Survey

Dear Parents and Guardians,

Students at Jean Vanier Catholic High School will be asked to be a part of the 2016/2017 Canadian Student Tobacco, Alcohol and Drugs Survey (CSTADS). This survey will gather details about health and behaviors amoung youth. This project has been approved by the Simcoe Muskoka Catholic District School Board (SMCDSB) and Jean Vanier Catholic High School. In Ontario, CSTADS is funded by Health Canada.

The CSTADS is a survey of students in Grades 7 to 12. It takes students 30 minutes to complete the survey. The survey will take place this Friday, April 28th in period 4 classes. Student names will not appear with the answers they provide on the survey and there is no way to link or identify a student to their survey results. Your child will be bringing home more detailed information regarding the survey today.

As a parent or guardian, you can decide whether or not you want your child to complete the survey. If a student is 18, they can decide on their own if they want to participate in the survey. Please indicate your consent in one of two ways:

1) Complete the consent form online at cstads.ca/permission. Use the online access code 122264 to complete the form.

OR

2) Complete the consent form that your child is bringing home today and return it your child’s period 4 teacher.
